Relationship to Viewer

I confirm that the parent/legal guardian information provided above is accurate to the best of my knowledge.

I understand that Cinematic Luxe Media will contact my parent/legal guardian directly and that my Youth Audience Viewing access will remain inactive until parent/legal guardian authorization has been received and verified.

I understand that I may not complete, forge, impersonate, or submit the Parent/Guardian Consent Form on behalf of my parent or legal guardian. False parental authorization may result in denial or immediate termination of Youth Audience access.

I understand that parental permission for Audience Viewing does not allow me to purchase a Cinematic Luxe paid subscription or enter Cinematic Luxe chatrooms or adult member community areas. Those services remain restricted to users age 18 or older.

Ages 13–17 — Parent/Guardian Approval Required

Viewers ages 13–17 may request limited Cinematic Luxe Audience Viewing access with verified permission from a parent or legal guardian. Youth Audience access does not include paid subscriptions, chatrooms, Premium/VIP areas, or adult community features.

Users under age 13 are not eligible for independent Cinematic Luxe accounts or Audience Viewing access through this process.
